• Evergreen Senior Care Services Information Office – Trained Resource Coordinators connect you to
  the programs and services you need throughout West Michiganat no cost.
• Grants for services and subsidized care available to families that qualify.
• Adult Day Services – Adult Day Health and Prime Time Programs for frail and health impaired adults.
  Social stimulation, friends, activity, health assessment, meals provided.
• In Home Care – Personal care, companionship, homemaking, errands, meal preparation offered in care recipient’s home.
• Home Delivered Meals – NSOA Meals on Wheels (Ottawa County Only) and Evergreen meals available.
