Evergreen Commmons is a nonprofit community center located in Holland, Michigan, serving Ottawaand Allegan counties.
Evergreen Senior Care Services (the Lakeshore hub for resources, care and support)

  • Evergreen Senior Care Services Information Office – Trained Resource Coordinators connect you to
    the programs and services you need throughout West Michiganat no cost.
  • Grants for services and subsidized care available to families that qualify.
  • Adult Day Services – Adult Day Health and Prime Time Programs for frail and health impaired adults.
    Social stimulation, friends, activity, health assessment, meals provided.
  • In Home Care – Personal care, companionship, homemaking, errands, meal preparation offered in care recipient’s home.
  • Home Delivered Meals – NSOA Meals on Wheels (Ottawa County Only) and Evergreen meals available.
  • Caregiver assessment, training and support groups. Caregiver library. Caregiver counseling services available.
  • Evergreen Bistro open to the public Monday through Friday from 11:30 to 1:00 p.m. Evergreen Commons is the Congregate Meal Site for Ottawa County seniors.
  • Support groups for caregivers, Parkinsons Support Group, Veterans’ Assistance
  • Care Coordinators are social workers who help you maintain your independence to live at home
    as long as possible by connecting you to services you may need.
  • Education on health and wellness topics and caregiving available.

  • Clinics – Foot Care, Hearing and Hear Aids, Blood Pressure, Eyeglass Cleaning and Repair
